Chronic Care Management (CCM) is a program provided by CMS to any Medicare or Medicare Advantage Member who has 2 or more health conditions that are chronic, such as Diabetes, Hypertension, Hypercholesterolemia, COPD, Chronic Kidney Disease, Peripheral Cardiovascular Disease, Depression, and many others. Participation is optional to any qualifying member, and we highly recommend it. It provides a monthly (or more if needed) phone check-up from our office to you. The purpose of it is for us to stay in touch with you, be aware of any issues you are experiencing, provide advice, connect you to social needs resources, help you maintain your care plan, address any needs or questions that have come up, make sure your medications are in order and accessible to you, etc.
