If you have provided us with an email address, a patient satisfaction survey will be sent to your inbox after your visit, once every 90 days, in which you may give us feedback regarding your experience in Evergreen, and the quality of care we have provided you. The survey data is collected and sorted, completely anonymously by the EMR software, and translated into a final score monthly. We encourage you to communicate your thoughts and feelings regarding our service to you via this tool.
